In 2012, Aspen Community Foundation contracted with CiviCore to build an online platform for its Cradle-to-Career initiative, a deployment that is now known as Neon CCM. Aspen Community Foundation implemented Neon CCM within a Case Management framework to connect community resources across western Colorado and to link schools, nonprofit organizations, students and families. The implementation centered on case management workflows and a community resource directory, enabling client intake, referral routing and service coordination capabilities consistent with Case Management applications. CiviCore was configured to capture participant records, track referrals and coordinate cross-organizational services to support education and family services use cases. Operational coverage focused on regional collaboration among schools and service providers in western Colorado, with governance emphasizing shared resource mapping and coordinated referral processes across participating organizations. The platform was used to improve service coordination and to centralize information flows between education, family services and community resource stakeholders.
