In 2021, Charter Healthcare implemented Netsmart myUnity within the Netsmart CareFabric platform. Configuration emphasized role based workflows for clinicians, intake teams and mobile caregivers to align documentation and verification processes.
The solution was deployed as part of the Netsmart CareFabric platform, enabling native interoperability across myUnity, Mobile CareGiver+ and Referral Manager modules. Operational coverage targets hospice, home health, palliative and complex care business functions, supporting referral intake through to mobile workforce execution and clinical care coordination.
Governance and rollout priorities focused on standardizing referral handling and establishing EVV compliant mobile visit verification policies. Implementation activities were organized to align clinical workflow owners, intake coordinators and mobile caregivers around consistent documentation, visit verification and referral escalation processes.
