In 2022, 3form deployed Gallup Q12 as its Employee Engagement application to support a 55 plus person glass manufacturing operation in Kernersville, North Carolina, aligning engagement measurement with frontline operations and manufacturing management. Gallup Q12 was used to instrument regular engagement measurement across shop floor teams and supervisory layers, providing a standardized Q12 survey cadence and engagement driver visibility for operations leaders.
The deployment configured Gallup Q12 capabilities common to the Employee Engagement category, including survey administration of the Q12 items, recurring pulse surveying, aggregated analytics dashboards, and structured action planning workflows for managers. Implementation emphasized manager-level coaching and transparency in results to close feedback loops, and the Gallup Q12 application was embedded into daily operational routines to surface engagement signals alongside performance metrics.
Governance and rollout were led by the Director of Operations and focused on cultural practices, clear expectations, and ownership for engagement and delivery metrics across the team. Following the Gallup Q12 program and parallel operational changes, 3form reported a 9.7 percent increase in the Gallup Q12 Employee Engagement survey score, a 500 percent increase in employee suggestions, a 30 percent decrease in turnover, a 10 percent increase in profitability, and an improvement in on time delivery from 30 percent to 99 percent, supported by a newly pioneered scanning process that improved WIP status and capacity visibility.
