In 2013, Moorings Park Foundation deployed TruBridge (Formerly Get Real Health) InstantPHR patient-portal solution under the Patient Management category to extend digital patient engagement for residents and authorized caregivers on its Southwest Florida campus. The rollout focused on the patient engagement and patient management process area, providing a unified resident-facing interface for appointment viewing, secure messaging, and remote health monitoring.
TruBridge (Formerly Get Real Health) InstantPHR was configured to expose scheduling and messaging capabilities to residents and caregivers, and to capture remote monitoring inputs for clinical review. Functional modules implemented included appointment viewing, secure clinician-patient messaging, and remote health monitoring workflows, each aligned to support care team coordination and resident outreach.
The implementation integrated with Aprima EHR to synchronize appointment and clinical communication information between the portal and the core electronic health record, supporting operational use by on-site staff and external providers. Operational scope covered residents, caregivers, nursing and clinical staff, with the portal serving as a patient management touchpoint across those user groups.
Softrim Corporation served as the implementation partner, supporting configuration, integration, and rollout activities described in the case study. The case study reports improved care coordination for staff and providers as a direct outcome of the TruBridge InstantPHR deployment.
