In 2017, El Centro Regional Medical Center implemented Oracle Cerner Siemens Soarian as its Electronic Health Record across core clinical departments. The deployment targeted inpatient and emergency workflows spanning Medical Surgical, Operating Room, Labor and Delivery, Obstetrics, Pediatrics, and a 20 bed Emergency Department in a geographically rural setting with constrained resources. Implementation emphasized point of care documentation and clinical task support for front line clinicians.
Oracle Cerner Siemens Soarian was configured to support clinical documentation, triage and assessment workflows, medication administration recording, IV documentation, wound care notes, telemetry event logging, and care plan management. The implementation incorporated category-aligned capabilities for nursing documentation, resident charting, and structured patient education content to standardize care delivery across departments. Clinical documentation and charting practices were a central focus during configuration to align with inpatient and emergency workflows.
Charting and documentation were performed using Sorian EMR software alongside Medhost for certain clinical processes, reflecting operational coexistence of those systems in daily care delivery. The Electronic Health Record supported multi-department coverage and clinician handoffs between Med Surg, OR, L&D, OB, Peds, and the Emergency Department, enabling consistent telemetry and medication workflows across those units. Integration details beyond co-usage were not specified.
Operational governance included a new graduate resident training program completed across all hospital departments, with specific training on charting using Medhost and Sorian EMR software and competency development for high acuity care. Trainees provided care to critically ill patients of all ages in a 4:1 patient ratio environment and subsequently secured a full time position in the fast paced Emergency Department. Training and role provisioning were used to embed clinical workflows and sustain EHR use in a resource limited rural hospital setting.
