In 2025, Southern Illinois University implemented D-ID AI Agents under the Chatbots and Conversational AI category to create simulated patient interactions for the SIU School of Medicine. The deployment produced real time virtual patients including a named agent 'Randy', designed to support medical student training and clinical skills practice.
The implementation used D-ID AI Agents' avatar infrastructure and conversational agent capabilities, combined with institution-managed prompt engineering and LLM integration to drive dialogue, nonverbal avatar behavior, and scenario orchestration. Functional components implemented include AI agent runtime, avatar rendering and speech synthesis, scenario scripting for clinical cases, and a prompt library maintained by SIU to shape clinical responses.
Integrations focused on connecting D-ID’s avatar infrastructure to institution-managed large language model endpoints and training scenario repositories, with SIU responsible for LLM integration and prompt version control. Operational scope centered on SIU School of Medicine learning and development in the United States, with a large scale pilot launched in March 2025 and ongoing expansion to additional cohorts and simulation scenarios.
Governance and rollout placed authorship of clinical prompts and scenario design with SIU faculty and educators, while D-ID provisioned avatar infrastructure and real time delivery, creating a clear split between vendor platform operations and institutional content governance.
