Illuminance AvantCare was used to centralize patient visit plans, clinical notes, medication records and rostering assignments, supporting day-to-day nursing operations and clinical handover. The rollout was Dynamics 365 and Power Platform based, providing a cloud-hosted application layer and Power Platform driven reporting and mobile forms to increase field visibility. This architecture delivered mobile access to patient records and real time reporting for managers, enabling supervisors to view staff locations, assignments and documentation status. Operational coverage centered on Chemo@Home Australia nursing teams and at-home chemotherapy patients within Australia, with governance changes to rostering approvals and clinical documentation workflows to support remote care delivery. The program increased field visibility and reporting while improving scheduling accuracy, qualification matching and frontline access to critical patient information during the COVID-19 period.
