In 2022, Presbyterian Health implemented HealthRules Payer as its Core Admin Processing System, replacing Cognizant TriZetto Facets. The implementation was executed under an enterprise PMO led RFP and vendor selection, intended to centralize payer core processing and adjudication across the health system.
The HealthRules Payer deployment concentrated on claims adjudication and core payer processing capabilities, with configuration work focused on adjudication rules, payment posting workflows, and member and benefits administration consistent with Core Admin Processing System functionality. HealthRules Payer was configured to support automated adjudication flows and batch and real time posting routines to align payer reconciliation and billing operations.
Integration work included the data integration of patient financials, coordinated alongside a $10m Epic Electronic Health Record upgrade to align clinical billing and payer financial flows. Operational coverage targeted hospital and physician group financial interfaces and revenue cycle touchpoints that support nearly 1500 physicians and nurses across the system.
Governance and rollout were managed by a Program Management Office that oversees a team of 50 project managers, 130 projects, and five portfolios representing over $400m in yearly revenue. The PMO centralized enterprise reporting and timesheet management for more than 1000 employees, led the HealthRules Payer replacement program, and executed staffing realignment that reduced contractor usage by 90 percent. The Facets to HealthRules Payer replacement was completed with zero system downtime and an estimated $5m in annual cost savings, and PMO staffing changes yielded an estimated $2.5m in annual savings.

The operational scope covered revenue cycle and finance departments and extended to payer facing reporting for employers and plans, consolidating data for both internal operational teams and external plan audiences.
Governance emphasized centralized reporting processes and automated distribution to reduce manual reconciliation between payer and provider views. The implementation delivered $3.4M in measurable value, achieved 90% automated reporting, and realized a nine month payback on investment, outcomes attributed to the combined use of MedeAnalytics RCM and payer reporting capabilities integrated with Epic and applied to revenue cycle and finance workflows.
