In 2018 Adult Medicine of Lake County implemented eClinicalWorks Population Health as part of its Electronic Health Record tooling for population health management. The practice is a 25 person primary care clinic in Mount Dora Florida and had been an eClinicalWorks customer for more than a decade, which anchored the deployment inside the existing vendor platform.
The implementation centered on population health capabilities within eClinicalWorks Population Health including Chronic Care Management CCM, Transition Care Management TCM, and HEDIS measure tracking. Configuration work focused on patient registries, care gap identification and closure workflows, enrollment and outreach pipelines for chronic care programs, and documentation workflows needed to support CCM and TCM billing and quality reporting.
